Lori Wernham is a family nurse practitioner (FNP) with a doctorate degree in nursing practice (DNP). Lori enjoys providing medical services to the community she was born and raised in.

Credentials:

·         Doctorate of Nursing degree from University of Wisconsin Eau-Claire in 2016

·         Accredited with the American Association of Nurse Practitioners (AANP-C).

·         9 years practicing emergency medicine and cardiac rehab services as a registered nurse.

·         BLS and Advanced Burn Life Support Certificate

·         Primary Prevention

·         Acute Care Condition

Lori has one daughter, Greta. Lori enjoys spending time outdoors on her hobby farm and riding horses.

Mike Borgen is a Family Nurse Practitioner who especially enjoys the bedside aspect of patient care.  Mike comes to Taylored Family Care with 15 years of Emergency Medicine background. Mike enjoys taking care of all aspects of family medicine, especially acute care visit needs.

Credentials:

·         Master’s of Nursing degree from Concordia University.

·         Accredited with the American Association of Nurse Practitioners (AANP-C).

·         15 yrs of ER experience as a Registered Nurse.

·         Advanced Cardiac Life Support, Pediatric Advanced Cardiac Life Support, Basic Life Support Certified.

Areas of Interest:

·         Acute Care conditions/injury/illness.

·         Procedures.

·         Health Promotion.

Mike is married to Kayla and they have three children together. Their son Kaden is the oldest, followed by their daughter Brynn, and finally their daughter Kinsley. Mike’s family enjoys traveling and spending time with family. Mike’s family also enjoys spending time outdoors. 

Jake was born in Medford and grew up in Westboro. He received a Bachelor of Science degree from the University of Wisconsin-Stout and is currently pursuing an MBA in Healthcare Management.

Prior to joining Taylored FamilyCare, Jake spent his career working in the health and wellness industry. Most recently, he provided clinical and operational oversight to Therapy services departments across three communities in the northwest region of Wisconsin. Jake provides oversight to the general operations of Taylored FamilyCare and works closely with local employers to promote affordable healthcare to our community. 

Jake married his high school sweetheart, Emily. They have two daughters and a son together (Tevy, Adley and Grady).

Jake has a passion for growth and enjoys the challenges and opportunities associated with the ever-changing healthcare industry. Jake also enjoys spending time with his family and being in the outdoors while hunting and fishing.

Born and raised in Medford, Jackie is married to her husband, Chris. Jackie attended Northcentral Technical College for Medical Assistant and graduated in 2013.

Prior to joining Taylored Family Care Clinic in November 2021, Jackie worked as a Medial Assistant in the outpatient and walk-in setting for seven and a half years.

Tiffany was born & raised in Medford, WI. In 2009, she graduated with an associates degree in Radiology from Lakeshore Technical College in Cleveland, WI. She is a registered X-Ray technologist through the American Registry of Radiologic Technologist (ARRT), and is also certified in CT and Mammography. Tiffany has over 11 years of hospital and clinical experience.

She is married to her husband Jeff and they have two children, Oliver and Eli. In her spare time, she enjoys traveling with family, especially going fishing in Canada.

Tiffany joined Taylored Family Care clinic in December 2021 as our Medical Assistant and X-Ray Tech.

Amy was born and raised in Rib Lake, Wisconsin. She attended Mid-State Technical College for Medical Assistant and graduated in 1998. 

Prior to joining Taylored Family Care Clinic in January 2023, Amy served for over 25 years in the Medical Field with 20 of those years being in the clinical setting. 

Amy is married to her husband Bob and they have three children; Austin, Madison, & Emma. In her spare time, she enjoys spending time with family, and friends, and being outdoors. 

Britney is a Medford native, after high school, she attended CVTC Eau Claire and obtained her degree in Phlebotomy. Britney has worked in the medical field for 8+ years of experience in the lab, medical reception, and processing. She joined Taylored Family Care in January 2022 as a Medical Receptionist. Britney is married to her husband Wes and together they have 3 young children. In her spare time, she enjoys spending time with her family, camping, fishing, and cooking.

DeAnna was born and raised in Wisconsin Rapids, Wisconsin. She received a Bachelor of Science degree from the University of Wisconsin LaCrosse in 1996. DeAnna and her family moved to Medford in 2007.

Prior to joining Taylored Family Care Clinic in June 2023, DeAnna spent 20 years in office work/customer service.  

DeAnna is married to her husband, Dan and they have three young adults (Jobe, Abe & Martha).

In her spare time, she enjoys spending time with family and friends, playing cards and board games, going to the movies and watching her daughter play softball.
