Taylored Family Care Clinic offers a direct pay model where every client knows prior to their appointment what they can expect for service and fees, without expensive insurance.

 

SERVICES

Taylored Family Care Clinic provides a comprehensive list of services for your entire family at affordable prices. From annual exams, acute care visits, pregnancy counseling, individual lab tests, Commercial Drivers License (CDL) physicals and certificates – Taylored Family Care Clinic has you covered. Services include but are not limited to:

  • Annual exams

  • Acute care visits (non-emergency) minor suturing, ear infections, sinusitis, strep throat, bronchitis, sexually transmitted infections, etc.

  • Family planning/contraception/pregnancy counseling

  • Peri-menopausal/menopausal care

  • Telephone Visits

  • Management of chronic conditions (diabetes, hypertension, thyroid, asthma, allergies, etc.)

  • Pediatrics, well child visits, WIAA sports physicals

  • Lab tests

  • X-Ray

  • EKG

  • Ultrasound

LEVEL 1 APPOINTMENTS: $95

  • Cold, sinus or flu

  • Bladder infection

  • Sore throat

  • Minor eye problems (sty, pink eye, etc.)

  • Ear infections

  • Impacted ear wax

  • Tic and insect bites

  • Acute bronchitis

  • Skin rashes (acute illness type)

  • Pregnancy testing

  • Middle school/High School Sports physicals

  • Camp physicals

LEVEL 2 APPOINTMENTS: $120

  • Established adult complete physicals

  • School/Insurance/Adoption physicals

  • Long term multiple medication management

  • Chronic illness management not covered in level 1

  • Review of tests, labs, imaging or consults from another clinic

LEVEL 3 APPOINTMENTS: $150

  • Pre-op Physicals

  • Multiple Chronic illness/condition management

  • Lyme work up

  • New patient physicals

  • Base fee for sutures and mole removal

Additional Services:

  • DOT Commercial Drivers License (CDL) Physicals: $95

  • Well-Child Exams: $105

  • Hormone imbalance/bio-identical hormones: $225

  • Pre-employment Physicals

  • Pre-employment / DOT drug screens

  • Pre-employment Fit Tests

  • Breath Alcohol Testing

PAYMENT POLICIES

  • This is a direct-pay practice

  • We keep costs low by having no billing or collections staff, therefore EVERYONE must pay their FULL BILL directly TODAY

  • Cash, checks, credit and debit cards are accepted

  • Writers of returned checks will be charged a $35 processing fee

  • Failure to pay your entire bill at the time of service will incur a $35 accounts receivable management fee. Future office visits will be required to be PRE-PAID (cash/credit/debit card only – no checks) BEFORE another appointment will be scheduled

  • We will provide an itemized receipt that you may use for insurance purposes if you wish

  • Medicare/Medical Assistance/BadgerCare patients, by law, may NOT seek reimbursement from insurance for care provided at Taylored Family Care Clinic, LLC

  • Failure to cancel an appointment for which you do not show up will require you to PREPAY (cash/credit/debit card only) your next office visit before another appointment is scheduled

***Telephone Visits are for basic acute care for common health conditions that might include Flu, Thrush, Strep, Heartburn, Eczema, & Pink Eye. Payment is collected at the time of service. (Major credit cards accepted). Our availability is subject to first come first serve and is not guaranteed, however, our goal is to get back to you within the business day. We will schedule a time when we are able to call you back. We reserve the right to require an in-office visit depending on the severity of the illness before giving our professional option for treatment.

TREATMENT LIMITATIONS AND LABORATORY TESTING

We will strive to limit any additional cost but not at the expense of appropriate medical care. Not all conditions can be completely managed or diagnosed by a primary care provider. Referral to a physician specialist or emergency room will be made when clinically indicated.

Specialized laboratory testing, pathology services or radiology services may be clinically appropriate. This testing will be ordered through an outside facility and the cost will be the responsibility of the patient.
